The Soap appears in The Eye and the Prime Object. It is briefly visible on the Sood’s computer when Joe Miller and Howard (The Weasel) Montague visit The Sood with the picture of Eddie and Mabel.
The powers of The Soap, if they exist, are completely unknown.
The Soap is a typical small bar of soap that is given away at motels and hotels all over the world. Since it is from the Sunshine Motel, it has the motel name and logo.
The soap wrapper can be obtained here. All that is needed is to add a small bar of motel/hotel soap (or something the same size and shape).

Went to find some of the paper to make the wrapper. As paulv70 said a "glossy" paper will have the same look as a real one. Also make sure it is a "low weight" paper, meaning it should slump over when held instead of remaining stiff. Also heard it referred to as "Flyer Paper".
Any printing place should sell you single sheets in lue of buying packages of 100.
